Application Procedure And Deadline

A complete application requires both electronically submitted and mailed materials. All must be received by THURSDAY, OCTOBER 1:

  1. Electronically submitted by noon October 1:

    Online application form
    Proposal narrative
    Budget (Excel template)
    Applications will be accepted on the CCH website (www.calhum.org) from September 1, 2009, until NOON on Thursday, October 1, 2009.

  2. Received by mail by 5 pm October 1:

    Previously completed work (DVD or CD)
    Mailed work samples must be received by the San Francisco office no later than 5 pm on October 1, 2009. Hand-delivered copies cannot be accepted.
